Long Wait Times for Doctors in Italy: How to Get an Appointment Faster Through the SSN

Due to long wait times in Italy’s National Health Service (SSN), patients are sometimes asked to wait several months for an appointment or test. However, if the deadline set for your referral is not met, you can contact your local health authority and request an alternative way to receive care. Find out what urgency categories are in place in Italy and how to protect your right to timely care

Getting an appointment with a doctor in Italy’s public healthcare system can take weeks or even months. Unaware of their rights, patients often give up waiting, turn to a private clinic, and pay for the consultation or examination in full themselves.

At the same time, Italy has priority categories for medical referrals that determine the maximum timeframe for providing medical care. If the public system cannot schedule an appointment within this timeframe, the patient has the right to contact the CUP or their ASL and request a so-called “percorso di tutela”—a special safeguard mechanism under which the service can be arranged at another public or accredited facility or, under conditions specified by regional regulations, on an intramoenia basis.

It is important not to pay for any private appointment expecting automatic reimbursement. First, you must follow the procedure established by your specific region and local health authority, and keep your referral and proof that an appointment within the required timeframe was unavailable.

What are the waiting times for different referrals?

The maximum waiting time depends on the priority class indicated by the doctor on the electronic referral. This code is determined based on the patient’s condition and the risk of health deterioration, so you cannot choose or change it on your own.

In Italy, four main categories are used:

1. U – urgente: the service must be provided as soon as possible, but no later than within 72 hours

2. B – breve: the consultation or examination must take place within 10 days

3. D – differibile: up to 30 days for the first specialized consultation and up to 60 days for a diagnostic examination

4. P – programmabile: a scheduled service must generally be provided within 120 days

These timeframes primarily apply to the first visit to a specialist or the first diagnostic test, not to follow-up visits or ongoing treatment. For scheduled services and certain types of care, the rules may vary by region, so it’s advisable to double-check the timeframe on your referral through the CUP or your ASL.

What should you do if the SSN cannot provide an appointment on time?

If you cannot book an appointment through the CUP within the timeframe specified for the relevant priority category, do not immediately pay for a private consultation. First, report the issue to your local health authority and ask them to arrange the medical service through the region’s designated safety net mechanism.

Recommended steps:

1. Check your referral. Make sure the doctor has indicated a U, B, D, or P code and has correctly specified the type of consultation or examination.

2. Contact the CUP. Ask them to check for available slots not only at the specific hospital but also at other public or accredited facilities within the service area.

3. Document the lack of an appointment. Save the referral number, the proposed date, screenshots, emails, or other proof that the service cannot be provided on time.

4. Submit a request to ASL. Ask them to activate the “percorso di tutela” because the maximum waiting time has been exceeded. Contact information and the request form are usually posted on the local health authority’s website.

5. Clarify the proposed alternative. The ASL may find a spot at another facility, engage an accredited provider, or arrange an appointment through another mechanism provided for by regional regulations.

It is important for the patient to accept the first available date that complies with the timeframe and territorial regulations. If a person voluntarily declines a suitable appointment due to an inconvenient time or a preference for a specific doctor or hospital, the right to request an alternative appointment may be forfeited. For example, Lombardy’s regulations stipulate that if an urgent referral cannot be fulfilled, the facility must seek another provider capable of delivering the service in a timely manner.

Can I see a private doctor and get reimbursed?

You cannot simply book an appointment at a private clinic, pay for the visit, and then automatically request a refund. For the costs to be covered by the public system, the alternative appointment must be arranged or approved through the ASL as part of the official safeguard mechanism.

If the SSN is unable to provide a consultation or examination within the timeframe specified in the referral, the local health authority must offer another available option. This could be an appointment at another public facility, at an accredited private center, or on an intramoenia basis. In such cases, the patient typically pays only the standard copay, unless they are eligible for a waiver.

Intramoenia refers to a fee-based appointment that a public hospital physician conducts outside of regular working hours. For the patient, it appears similar to a private consultation but takes place through a public healthcare facility. If the permissible timeframe is exceeded, the ASL may use this format to ensure timely service delivery without requiring full payment from the patient.

It is important to first contact the CUP or ASL, submit a request to activate the “percorso di tutela,” and wait for official instructions. If a person independently chooses a private doctor without prior approval, reimbursement may be denied. The specific application procedure, list of required documents, and payment rules must be verified with your local ASL, as they vary by region.

What is important for foreigners in Italy to keep in mind?

Foreigners registered in the Italian SSN system are subject to the same appointment rules and priority categories as other patients. However, in practice, difficulties often arise due to the language barrier, varying regional procedures, and a lack of knowledge about which institution to contact.

To protect your right to timely care, you should keep your referral, appointment number, proposed appointment date, screenshots from the appointment system, and correspondence with the CUP or ASL. When making a request, you can use the phrase “richiesta di attivazione del percorso di tutela per superamento dei tempi di attesa”—a request to activate the protection mechanism due to exceeding the waiting time.

You should also be aware that the ASL may offer an appointment not at the nearest hospital, but at another facility within the designated territory. Refusing an appointment solely because of an inconvenient location or a desire to see a specific specialist may complicate any subsequent appeal.

Before filing a complaint, check the website of your ASL or regional health authority. They typically publish the CUP contact information, the procedure for filing a complaint, the form to activate the “percorso di tutela,” and the rules for obtaining an alternative appointment.

When choosing a clinic and preparing documents for treatment abroad, it is easy to get lost in the requirements of each individual country - because the list of certificates, the amount of insurance coverage and even the wording in the letter from the doctor differ depending on the consulate.

The treatment guide from Visit World solves this very problem: after you indicate your citizenship and the country where you plan to go for treatment, the service creates a personalized guide with a full list of documents, an explanation of the rules for filling out the visa application form and step-by-step instructions - from the first appointment at the clinic to passing the visa interview. It also includes practical advice from people who have already gone this way, so you will know in advance what to pay attention to when applying. This is especially valuable when time for treatment is short, and the price of an error in the documents is lost weeks of waiting for a new date.

Order a guide and choose the best clinic for treatment in any corner of the world.
