Treatment in Angola for Madagascar citizenship

Documents required

1. The applicant's letter addressed to the consulate of Angola, in which he/she asks for a visa for treatment, with a notarized signature, which must be certified by the same consulate.
2. A completely filled-in questionnaire, done online and printed out;
3. Passport valid for more than 18 months and 2 free pages for affixing a visa;
4. two (2) copies of the main pages of the passport and pages containing information on migration;
5. three (3) personal photos, in color, recent, original, on a white background, and without sunglasses (except for a justified medical recommendation). Photos must be pasted on the application;
6. Certificate of no criminal record, authenticated by the consulate (original and copy of the authenticated certificate of no criminal record);
7. Declaration of commitment to compliance with the laws in force in the Republic of Angola (with a notarized signature certified by the Consulate);
8. A document confirming the passing of a medical examination, duly certified by the hospital;
9. Medical opinion issued by the relevant medical center, certified by the Consulate (original and ocopy of the certified opinion);
10. Means of subsistence equivalent to USD 200.00 for each day of stay in Angola (this amount must be confirmed by a bank statement certified by a white seal or an oil seal of the same banking institution);
11. An official document issued by an organization overseeing a medical facility that will provide assistance or serve the applicant in Angola;

FOR minors (in addition to the above requirements):

• Travel authorization issued by guardians, with notarized signatures;
• Term of responsibility of guardians, with notarized signatures;
• The original and a copy of the passport or ID card of the accompanying person.

OBS: 

The answer is given within a maximum period of 60 working days;

The visa must be used within 60 days of its issuance and is valid for a 180-day stay in Angola with multiple entries; it can also be extended in case of valid reasons;

Evidence of means of livelihood must be presented and checked at the border crossing point.

This visa is issued for medical treatment in public or private hospitals.
