Necessary documents for obtaining a medical Schengen visa to Portugal.
When applying for a visa to receive medical assistance in Portugal, the following additional documents must be submitted:
Local medical opinion. Issued by a doctor/clinic/hospital in your country confirming that you need treatment in Portugal.
A medical certificate from a hospital or doctor in Portugal confirming the date of your appointment and your medical condition. He must also confirm that the hospital can provide the specific treatment and that you will be accepted accordingly.
Confirmation of the financial agreement.
Official confirmation of treatment/appointment by the receiving medical institution (hospital/clinic).
Applications for extension of stay are submitted at any Directorate or Regional Delegation of SEF using a standard form signed by the applicant or his/her legal representative, or online, with the necessary documentation, namely:
The application is submitted in person (on a standard form), signed by the applicant (if the applicant is a minor or disabled, the form is signed by his legal representative).
Two recent, identical photos in color with a white background that are easily identifiable.
Passport or other valid travel document.
If the applicant is a minor or a person with physical disabilities, the application must be submitted and signed by their legal representative.
Evidence of sufficient means of subsistence.
Evidence that the applicant has adequate housing.
Criminal record check permit if the stay is expected to be more than 90 days (except for persons under 16 years of age).
A document issued by an official health care institution certifying that the applicant continues to receive medical care and has a place for inpatient or outpatient treatment.
Applications submitted online do not exempt the applicant from collecting biometric data and pasting the SEF sticker by registering at one of the service bureaus.
The validity period of a temporary stay visa, including the corresponding extension of stay, cannot exceed one year.
Applications for the extension of the period of stay will not be accepted if they are submitted more than 30 days after the end of the authorized period of stay.
The following administrative offenses apply: Article 192 of the Law on Foreigners (Illegal Stay); Article 197 of the Law on Foreigners (Declaration of Prohibition of Entry); Article 198 of the Law on Foreigners (Unauthorized professional activity); Article 199 of the Law on Foreigners (Without Travel Document).
