Treatment in Switzerland for Philippines citizenship

    If you are willing to move to Switzerland to receive treatment and remain in the country for less than 90 days, you will need to obtain a short-term type C visa. 

    —

    Necessary documents:

• One Schengen visa application per person, carefully completed in German, French, Italian or English and signed.

    Each visa application must contain signatures in 2 places: 1) paragraph № 37, 2) in the end of page 4 after the text of the declaration;

    Visa applications of underage people (less than 18 years old) have to be signed by one of the parents of legal guardians. 

• Two current passport-size photographs (one photograph must be glued to the visa application and the other attached to it);
• Valid foreign passport;

    Passport has to be valid for at least three months after planned return back from the Schengen Area and has to have at least 2 blank pages.

    The date of issue must not exceed 10 years. 

• Copy of the first page of foreign passport (the page with personal information and a photograph);
• Copies of all pages with Schengen visas that were issued during the last 24 months and copies of all pages with valid visas for other countries;
• National passport;

    Applicant has to provide an original and a copy of their passport.

• Medical insurance policy;

    Medical insurance policy has to cover all expenses that may occur in connection with repatriation for medical reasons, urgent medical intervention, medical treatment or death while in the Schengen Area;

    The insurance policy has to be valid in all countries of the Schengen Area throughout the entire time of stay and the insurance payment must be at least 30,000 Euros.

    For children (under 18 years old):

1. Notarized travel permit for children travelling without parents of accompanied by one of the parents;
2. The child’s permission to leave must be notarized and provided in the original with a photocopy;
3. In case a child lives with only one guardian, a birth certificate, court decision on sole custody, death certificate or other documents must be provided;
4. The child's travel permit and other documents must be translated into German, French, Italian or English.

• Evidence of employment and income

    All applicants must provide proof of livelihood in their country of residence. If the applicant is unemployed, a written application (signed) must be provided explaining the applicant's financial status.

    For employees: certificate from the place of work indicating the position and salary of the employee for the last 6 months, date of employment, duration of leave. The certificate must indicate the name and position of the person who signed it, and contact details of the company;

    For private entrepreneurs: license / documents on registration of the enterprise from the competent Ukrainian authorities or the latest tax return;

    For pensioners: pension certificate;

    For schoolchildren and students: certificate from the place of study or consent of the educational institution regarding the absence of the pupil / student during the educational process;

    Additionally, if necessary: ​​documents confirming the ownership of land, house, car, etc., to confirm the financial status of the applicant.

    All documents proving employment and income of the applicant must be provided with a translation into German, French, Italian or English.

• Proof of financial capability;

    All applicants must document that they have sufficient funds to cover the costs of the planned trip and to return home;

    Examples of proof of financial capability: bank statement with transactions for the last 3 months (original or copy with the bank's seal), certificate from the bank with information about the available amount on the applicant's account (original) or traveler's checks indicating the applicant's name and signature copies);

    Documents confirming the applicant's financial capability are provided without translation.

• Original (copy can be accepted only in urgent cases) invitation from the clinic or medical institution confirming full or partial payment for future treatment.

    The Swiss Embassy reserves the right to:

• Require further additional documents (Article 21/8 of the Schengen Visa Code);
• Send visa applications for decision to the competent Swiss authorities;
• Invite the applicant for a personal interview (Article 21/8 of the Schengen Visa Code).
