Required documents for a medical Schengen visa to Austria
When applying for a visa for medical treatment in Austria, you must provide the following additional documents:
- The local medical certificate. Issued by a doctor/clinic / hospital in your country which is confirmed that you need treatment in Austria. A medical certificate from a hospital or doctor in Austria confirming the date of your appointment, as well as your medical situation.
- A receipt for payment of medical fees.
- The official confirmation of treatment/appointment at the host medical facility (Hospital/Clinic) in Austria.
- The confirmation that you have sufficient financial resources to pay for medical expenses and other related expenses during the entire treatment period.

Documents on the patient's employment status:
If employed:
- An employment contract.
- An approved email from your company.
- Settlement receipts for the last 6 months.
- Income Declaration.

If you are self-employed:
- A copy of your commercial license.
- Bank Statement of the company for the last 6 months.
- Income tax return (ITR).

- The confirmation of admission to the University.
- A letter of no objections from the University.

Additional requirements for minors:
- The birth certificate. Provide this help in the original with a copy.
- A consent letter. An official document from the second parent confirming that he / she doesn’t object to their child traveling only with you, if possible.
- From both parents. An official document from you and your child's second parent certifying that they have no objections to your child traveling with a third party, if possible.
- A death certificate of one of the parents. In case one or both parents have died.
