Learn more about who is eligible for free family insurance in Germany and what restrictions to consider
Germany offers one of the most developed healthcare systems in Europe, and an important part of this system is the possibility of free health insurance for relatives of the insured person. The Familienversicherung program operates within the framework of state health insurance (Gesetzliche Krankenversicherung) and allows you to include relatives in the policy without additional contributions.
Who exactly can take advantage of this option, what age and income restrictions exist, and what you should pay attention to — we will tell you more in this article.

What is Familienversicherung and how does family insurance work in Germany?
Familienversicherung is a mechanism whereby family members of a person insured under the state health insurance system receive medical coverage without paying separate insurance premiums. In fact, insurance costs are covered by the primary payer — an employee or other person who makes contributions to the state treasury. Relatives included in family insurance have access to the same services: doctor consultations, inpatient treatment, diagnostics, and prescription medications.
This model is unique to the public sector. Private insurance companies do not offer such a program — each family member pays for their policy separately. Therefore, Familienversicherung often becomes a significant financial argument in favor of state insurance for families with children or with one working partner.

Who can be included in family health insurance in Germany?
The list of persons eligible for free family insurance is defined by law and covers immediate relatives. First and foremost, this includes the spouse or registered partner of the insured person, provided that this partner does not have their own compulsory insurance. Children can also be included in the program: biological, adopted, foster, as well as stepchildren who live in the insured person's household.
In some cases, grandchildren who are supported by the insured person may also be included in Familienversicherung. The main condition for all categories is permanent residence in Germany. Persons who are permanently outside the country are not eligible for family insurance.
Up to what age are children eligible for Familienversicherung?
The age limits for family insurance for children depend on their employment and education status. All children under the age of 18 are unconditionally entitled to free medical coverage — there are no additional requirements in this case.
After the age of 18, the situation changes. Children who are not working retain their right to Familienversicherung until the age of 23. Those who continue their education — attending school, undergoing vocational training (Ausbildung), or attending university — can remain in family insurance until the age of 25. After reaching this age, it is necessary to take out your own insurance policy, even if you are not employed.
Income restrictions for family insurance participants
One of the key conditions for participation in Familienversicherung is compliance with the established income limit. A family member who is to be included in the insurance must not have a monthly income exceeding €565. For persons employed under Minijob (mini-job) conditions, the threshold is slightly higher — up to €603 per month.
When calculating income, all sources of income are taken into account: wages, income from self-employment, rental payments, investment income, pension payments, and alimony. It should be emphasized that self-employment should not be the main activity of a person applying for family insurance. Another mandatory condition is the absence of compulsory or private health insurance.
The impact of one parent's private insurance on a child's coverage in Germany
Special attention should be paid to situations where one parent is insured in the private system and the other in the public system. In such cases, the child's right to Familienversicherung may be limited. In particular, difficulties arise when the father or mother with private insurance has a higher income than their partner in the public system.
In such circumstances, it may not be possible to include the child in free family insurance, and a separate policy — public or private — will need to be taken out for them. This situation requires individual assessment, so it is recommended to consult with the insurance company (Krankenkasse) in advance to find out the coverage options for the child.
